Samuel Wayne Ellsworth, son of Darlene Montgomery of Dexter and the late Harold Samuel Ellsworth, was born on June 9, 1958, and died in the St. Francis Medical Center at Cape Girardeau on Saturday, July 22, 2006, at the age of 48.

Samuel was baptized into the Church of Christ at age 11 and was a resident of Malden.

Survivors include his mother and step-father, Darlene and Hollas Montgomery of Dexter; and one brother, Terry Ellsworth of Gideon; and two nieces, Michelle Ellsworth and Sara Jackson, both of Gideon.

Visitation was held from 12:30-2 p.m. Monday, July 24, 2006, at the Rainey-Mathis Funeral Home in Dexter.

A memorial service was conducted at 2 p.m. in the Rainey-Mathis Funeral Chapel with Stan Dauck and Barry Montgomery officiating.
